DEMENTIA CARE Lithgow Aged Care Ltd has two dedicated and separate dementia specific wings. The facilities are a secure area in which residents with dementia can be accommodated. Residents are able to wander freely in this facility and have access to a garden space as well.
Identification of need Staff at Lithgow Aged Care Ltd identify the needs of people with dementia by meeting with family members, Medical Officer, talking to the resident/other carers and reading any relevant documentation to gain a holistic picture of the resident and their individual needs. This information enables staff to develop a care plan for each resident with further assessments area reviewed every three months, or when required. To deal with complex needs allied health professionals are involved and specialised equipment is also provided. Activities Activities are client centred and provided on a daily basis. These include a hugely successful programme called the Sunshine Singers which involved forming a choir with our residents (both resident’s with a diagnosis of dementia and those without this diagnosis) and local school children combined.
